5. PLANNING
a. Planning questions require prioritizing nursing diagnosis, determining goals and outcome criteria for goals of care, developing the plan of care, and communicating and documenting the plan of care.
b. Regarding nursing diagnoses, remember that actual client problems rather than potential or at risk client problems will most likely be the priority.
c. Remember that this is a nursing examination and the answer to the question most likely involves something that is included in the nursing care plan, rather than the medical plan.
PRACTICE QUESTION: The Nursing Process – Planning
A nurse develops a plan of care for a client with a cataract. Which nursing diagnosis is the priority?
1. Fear related to lost of eyesight
2. Social isolation related to decrease ability to mobilize in the community.
3. Disturbed Sensory Perception (visual) related to ocular lens opacity.
4. Risk for injury related to decrease vision.

Test-Taking Strategy: This question relates to planning nursing care and asks you to identify the priority nursing diagnosis. Use Maslow’s hierarchy of needs theory to answer the questions. Remembering that physiological needs are the priority will direct you to option 3. Although Risk for Injury is a potential rather than an actual problem, according to Maslow’s hierarchy of needs theory, safety is the second priority. Fear and Social Isolation are psychosocial needs. Remember planning is the third step of the nursing process.

NOVEMBER 2008 NURSING BOARD EXAM BULLETIN
At least 39,455 or 44.51% out of 88,649 examinees that took the November 2008 Nursing Board have passed, the Professional Regulatory Commission has announced.
Jovie Ann Alawas Decoyna of Baguio Central University topped the board, with 89 percent, the PRC said.
Xavier university ranked number one among the schools whose students took the board exams, the results of which were released by the PRC Friday.
The Nursing Regulatory Board (NRB) is headed by Carmencita Abaquin. Members of the NRB include Yolanda Arugay, Betty Merritt, Leonila Faire, Perla Po, Marco Antonio Sto.Tomas, and Amelia Rosales.

NOVEMBER 2008 RESULTS BULLETIN : Result will be released within the next 10 Days.
The Board of Nursing started its quarantine today, February 08 2009. Deliberations about the passing rate, top performing schools and the topnotchers are already finished by the BON and the PRC rating and statistics division.
The Quarantine is performed when the results is to be release as to prevent the BON from influencing any aspect of the results.
Results will be release anytime within the next 10 days. Passing rate is at a usual percentage of 40s but can still change.
Results will be release here 1 day before the national release.
